Our patient information form is conveniently available to you online (above).  We encourage you to fill it out prior to your appointment.  This will save you a considerable amount of time prior to your eye examination.
Simply click on the form to download and print.  Please bring with you the completed forms to your appointment.

For existing patients:
For returning patient, if you would like to update your medical history or personal information, please bring in an Updated Patient Form to your next appointment, or simply contact us by phone to make the changes.


WHAT TO BRING WITH YOU
Please bring the following items with you to your appointment:

• ​Current insurance card, Medical and Vision.
• If you wear glasses, bring in your current or old glasses
• If you wear contact lenses, bring in your contact lens boxes and come in wearing your contact lenses.  Please bring your back-up glasses if you have one.
